Forgotten Mandatory Effects Penalties (DD Survivor, Sangan)

I judged at a Regionals recently and after a discussion with the HJ I penalized both players for missing the mandatory effect of Sangan. Sangan's owner because he forgot Sangan's mandatory effect and his opponent, for not reminding him. I was not Procedural Error - Major - Irrepairable game state. It was judged that both players get Procedural Error - Minor - Forgetting Sangan's effect, and Sangan's owner is penalized more in reality because he did not get to search for Sangan's effect.

Last night during a local game I had a slightly different scenario. Player A forgot about his DD Survivor's effect during his opponent's end phase, then drew for his turn, then tried to return DD Survivor. I gave the same ruling as for Sangan: both players PE - Minor. The DD Survior player is penalized more in that DD Survior is now stuck in the remove-from-play area.

A related question is that the DD Survivor issue came up while I was table judging the final match. Should I get a Judge Error -- Asleep at the Wheel for not reminding the player myself, or should I leave it on the players to handle those kind of situations that may lead to penalities, just not irrepairable game states?
