illegal play caught too late

Hi, I was wondering what would be the correct assessment of this situation. I'm giving an example with an illegal play in Yu-Gi-Oh! but it could be in any other game.
Player A activates Soul Exchange in his Main Phase 2 (after declaring an attack).
Neither player figures out the mistake at this point.
Player A passes his turn and player B draws his card for the turn.
At this point player B figures out the mistake and calls over a judge.
Assuming Player A did the illegal play unintentionally and Player B did not intentionally wait until the game state was unrepairable to call a judge in order to look for a quick way to win, what would be the correct penalty for each player?
Player A argues that it is both player's responsability to maintain a correct game state.
I would have given a Game Loss to Player A for Procedural Error - Major and a Warning to Player B for Procedural Error - Minor. Would this be correct?
