UDE Number cards..where to get them?

Hello,People...i have a question regarding those little cardboard cards..

Im new at judging (even newer at PM or TO),so my main question is about the cardboard cards that are given to issue new UDE numbers. I live outside the states,so i was wondering if i need to have a valid US adress to apply for those cards or if they can be shipped outside the states??
