Reversal Quiz

I have 3 questsions.

A. With the card I pick up. Does it 1. Go to grave. 2. Go in hand. 3. Is put back on the deck.

B. If I am understanding the ruling correctly when Fuhma Shuriken (sp?) and/or Black Pendant are out on the field, (set or equipped)
and I activated Reversal Quiz, they take damage AFTER the life point switch right?

Example: I have 400 life points left. I have 1 Black Pendant set Reversal Quiz and Dark Hole in my hand. If I played Reversal we would switch lp. Me: 8000 Him: 400 (After switch) He would take the 500 after Reversal Quiz killing him right?

C. Do I have to have a hand when I play Reversal Quiz? I know it says something about hand and field (Dont have the card with me at the moment) I recall a earlier ruling that said I had to have a card in my hand in order to play Reversal Quiz. Is this still true?

A. It goes back to the top of the Deck. You only pick it up and reveal it to both players to show whether you got it right or wrong.
B. Black Pendant would inflict the 500 damage after Reversal Quiz resolved, I think. It should wait until after resolution anyway.
C. You have to have at least one card in your hand or field to activate Reversal Quiz. Sending your hand and field to the Graveyard is a cost of activating Reversal Quiz.
