Situation:
Player A has f/u ATK pos Gemini Elf and a f/d Call of the Haunted Set 2 turns ago.
Player A has a BEWD in his Graveyard

Player B has a Set Man Eater Bug

Play:
Player A suspect that the Set monster is a Man Eater Bug, so he decides not to use CotH on BEWD yet.
Player A attacks the Set monster with Gemini Elf.
Player B Man Eater Bug's effect activates, targets Gemini Elf, and destroys her.

Questions:
1) When can you activate Call of the Haunted to bring back BEWD for a direct attack?

2) Can you activate CotH during Step 6 of the Damage Step?

3) If you decide not to activate CotH on Damage Step, what is the next Step? Does the Game Mechaninc goes to Battle Step, or does it go to End Step since there is no more monster on the field.

4) Can you activate CotH during End Step? If you can, can you go back to Battle Step?

No phase ends without the permission of both players. Once a battle is complete, you don't have to immediately go to the next battle, you can activate Spell-Speed 2 cards as you would in any other phase. Then it proceeds as normal, you may declare an attack with any monster that hasn't declared an attack yet.

2) Can you activate CotH during Step 6 of the Damage Step?
No, it is not allowed in the Damage Step at all, and you would have to wait until you go back to the Battle Step.

3) If you decide not to activate CotH on Damage Step, what is the next Step? Does the Game Mechaninc goes to Battle Step, or does it go to End Step since there is no more monster on the field.
You would go back to the beginning of the Battle Step. You could then activate Call of the Haunted, Special Summon a monster, and declare another attack of you so wished.

4) Can you activate CotH during End Step? If you can, can you go back to Battle Step?
Yes you can. However, you will not be able to go back and declare another attack.

The mechanics of the Battle Phase are, that you keep cycling from the Battle Step through the Damage Step and back to the Battle Step until you have decided not to attack any more. Once you declare the end of your Battle Phase (you do not wish to attack any longer) you enter the End Step, in which Spell/Traps can be activated (like any other Step) but once it is complete, your Battle Phase will be over.

In otherwords, once you enter the End Step, you cannot go back to the Battle Step.

Hope that helps

Thanks for the replies. I wanted to see the Game Mechanincs of YGO in terms of Battle Phase. The one in the rule book is not specific enough to cover all situations. Please verify if the folowing Game Mechanics are plausible:
  1. Battle Phase: You may not enter this phase if you are the starting player. If you do not to enter Battle Phase, you proceeds to End Phase. You may enter this phase even if you don't have any monster in your field.
    1. Start Step: Declare that you are entering the Battle Phase. Spell Speed 2 can be activated on this Step.
    2. Battle Step:
      1. Step 1: You can activate Spell Speed 2 here. (Example: You can activate CotH here).
      2. Step 2: Decide if conducting battle. If you are NOT conducting battle, got to End Step.
      3. Step 3: Select and announce 1 monster to attack with, and declare 1 of your opponent's monsters your target (the monster you wish to attack). Spell Speed 2 can be activated on this step.
    3. Damage Step: Use Kevin's Chart here.
    4. Go back to Battle Step:
    5. End Step: You can activate Spell Speed 2 here.
